SERVICE EMPLOYEES BENEFIT FUND DEATH BENEFIT Service Employees Benefit Fund (SEBF) provides a Death Benefit for eligible members of SEIU Local 200United and 1199SEIU. Death Benefits are paid directly from Service Employees Benefit Fund. As an active member, benefits are payable to your named beneficiary if you die while eligible for Plan benefits. This benefit provides extended coverage if you are a totally disabled member. Coverage will be extended and benefits will be paid according to these rules:
To be sure that your Death Benefit is paid to the person of your choice, you need to file a beneficiary card with the Fund office. If you have not completed an enrollment card naming a beneficiary, or if you want to change your beneficiary, you can get a card by calling the Fund office at (315) 424-1754. List all beneficiaries on the card if you want your Death Benefit to be paid to more than one person. Only the beneficiary or beneficiaries named on your card will be entitled to benefits. Whoever you name as your beneficiary is confidential. No information will be given to anyone over the telephone about your beneficiary. |
